Delivery Team Approval Form

Complete this form before any new delivery team is used.


This form is used to check:


  • insurance

  • PPE

  • van / equipment

  • ability to carry out Academy deliveries

  • willingness to follow Academy rules


Only teams that meet the minimum standard should be approved.
